Certification of Disability for Special Dietary Needs
A "Certification of Disability for Special Dietary Needs" form must be completed every year for students needing this service.
Forms may be obtained from the school nurse on each campus or by using the link below. Please be sure to have all sections completed.
Requests with incomplete forms will not be honored.
Your physician must list substitutions to be served in the place of items needing to be omitted.
